Report Identity Theft


If you are concerned that you or your business have been the victim of identity theft, you should act right away. Follow the steps below to begin the recovery process.

STEP 1

Report the theft to the Federal Trade Commission. Get a recovery plan and put that plan into action.

STEP 2

Report the theft to your local law enforcement agency.

STEP 3

If an individual has submitted unauthorized filings on behalf of your business or if your name and address were used without your consent in connection with a RI business, you can file a complaint.

  • If your legal name and/or residential address was used to form a business without your knowledge or consent, complete the Affidavit of Unauthorized Formation, being sure to include the report number from the report you filed with law enforcement (see Step 2) and submit it to the RI Department of State by email ([email protected]), mail or in-person delivery at 148 W. River Street, Providence, RI 02904.
  • If your business has been the victim of identity theft and an annual report or other filing was submitted to change officers, directors, partners or managers without the entity’s consent or knowledge, complete the Affidavit of Unauthorized Reporting and submit it to the RI Department of State by email ([email protected]), mail or in-person delivery at 148 W. River Street, Providence, RI 02904.
  • The completed affidavit will be used as evidence to begin the remediation process.
